Identification and control of an outbreak of ciprofloxacin-susceptible EMRSA-15 on a neonatal unit

この論文は、英国流行型メチシリン耐性黄色ブドウ球菌-15（EMRSA-15）のシプロフロキサシン感受性株による新生児病棟におけるアウトブレイクの同定と制圧を報告している。入院時にシプロフロキサシン含有培地を使用してすべての乳児をスクリーニングしていたが、アウトブレイク株は検出されなかった。最初に判定された症例は、脛骨骨髄炎と多発性皮下膿瘍を伴うMRSA菌血症を発症した早産児であった。次に、臨床的感染症を認めなかった2例目の小児の鼻咽頭分泌物からアウトブレイク株が検出された。シプロフロキサシン非含有培地を使用した新生児病棟の全患児に対するスクリーニングから、新たに2例の保菌乳児が判定された。全4患児の分離株はEMRSA-15、spa型t022、SCCmec IVであり、Panton-Valentineロイコシジン（PVL）陰性、パルスフィールド・ゲル電気泳動法で同一パターンであり、検査したすべての非β-ラクタム系抗菌薬に感受性であった。共同搾乳室の48の環境部位の4カ所から、アウトブレイク株の培養を行った。搾乳室へ出入りする母親の非監視下の動きが、アウトブレイクに寄与していた可能性がある。感染対策として、患児の集団隔離、環境清掃の改善、手指消毒の徹底、および母親の教育を実施した。シプロフロキサシン感受性株（市中獲得型MRSAを含む）保有率が増加している環境でのMRSAスクリーニングでは、シプロフロキサシン含有培地の使用に注意が必要である。

欧米での増加が問題となっている市中獲得型MRSA（CA-MRSA）は、SCCmec IV、PVL陽性などの特徴があるが、非β-ラクタム系抗菌薬感受性である場合が多く、CA-MRSAが問題になるような環境にあっては監視培養検査においてもどのような選択培地を使用するか注意が必要である。積極的監視検査を実施する際も検査方法の限界を検討しておくべきである。わからないときは認定臨床微生物検査技師さんに教えてもらいましょう。
