Control of the duration of urinary catheterization: impact on catheter-associated urinary tract infection

カテーテル関連尿路感染は最も頻度が高い院内感染の1つである。カテーテル関連尿路感染の予防にとって、カテーテル留置期間の短縮は重要な介入であると考えられる。著者らの大学病院の5部署で前向き時系列・非無作為化介入研究を実施した。看護師から医師へ不必要な尿路カテーテルを抜去するようにカテーテル挿入4日目以降から毎日注意喚起することの影響を明らかにすることを目的とした。この介入により5部署中の2部署でカテーテル留置期間が有意に短縮した。全5部署のカテーテル留置患者の遅発性カテーテル関連尿路感染の発生頻度は患者100例あたり10.6例から1.1例に低下して（P＝0.003）、発生率は延べ1,000カテーテル・日あたり12.3から1.8まで減少した（P＝0.03）。ロジスティック回帰分析により、カテーテル留置期間およびカテーテル交換の繰り返しが遅発性カテーテル関連尿路感染と関連があることが示された。この研究では遅発性カテーテル関連尿路感染の発生頻度を簡単な方法で低下させることが可能であることを示している。
