A longitudinal study of Acinetobacter in three Australian hospitals

アシネトバクター属菌は、特に抗菌薬耐性の増加により、近年では院内感染病原体として著しく増加している。この研究の目的は、メルボルンの3病院のアシネトバクター陽性率と抗菌薬感受性の変動を記述することである。著者らは5年間の微生物学検査記録を後向きにレビューした。四半期ごとに10,000退院あたりでアシネトバクター臨床分離株が新規に同定された患者数を算出した。その他に収集した情報は、抗菌薬感受性パターン、患者年齢、性別、入院日数、および病棟の種類［集中治療室（ICU）または非ICU］である。同定率は2病院で大幅に上昇したが、3番目の病院では大幅な上昇はみられなかった。1病院では、同定数の増加は抗菌薬耐性と関連していた。初回分離株のほとんどは、患者のICU在室中に同定されていた。呼吸器検体からの分離株が最多であったが、血液由来の比率も極めて高かった。この研究はメルボルンの2病院でアシネトバクター属菌が院内感染病原体として定着したことを述べるとともに、将来への警鐘を鳴らすものである。

監訳者コメント：
わが国では多剤耐性緑膿菌（multi-drug-resistant P. aeruginosa；MDRP）が注目されているが、高度耐性アシネトバクターにも注意を払っておく必要がある。アシネトバクターも緑膿菌と同じくグルコース非発酵グラム陰性桿菌であり、容易に薬剤耐性を獲得するなど、共通点が少なくない。
