Molecular characterization of an acute hepatitis A outbreak among healthcare workers at a Korean hospital

韓国の若年成人のA型肝炎ウイルス（HAV）抗体保有率は低い。2005年の5月から7月の間に、病院の集中治療室（ICU）の医療従事者から17例のHAV感染が報告された。アスパラギン酸アミノトランスフェラーゼ（AST）値およびアラニンアミノトランスフェラーゼ（ALT）値の上昇を示す内科・外科ICUの全患者について抗HAV IgM抗体の有無を調べ、初発症例の疑いのある2例と接触した全医療従事者でAST値とALT値のスクリーニングを実施した。アウトブレイクを確認後、医療従事者の血液中のHAVの分子亜型を調べた。初発症例および伝播経路を同定し、アウトブレイク制御のための介入策を実施した。医療従事者の17例は22～32歳の看護師13例と医師4例であり、いずれも研究期間中に急性HAV感染に罹患していた。HAVの伝播方法は、下痢のある長期臥床患者からの糞口感染経路が考えられた。医療従事者は全例が抗HAV IgM抗体陽性で、うち8例はHAV RNA陽性であった。各分離菌株のVP1-2A領域の解析から、5株が遺伝子型IA、その他の株では遺伝子型IAとIBの同時循環（co-circulation）であることが示された。今回のHAVアウトブレイクにより、病院内の標準的感染制御予防策の重要性が明確になった。患者の血液の分子学的検査は、病院でHAV感染のアウトブレイクが疑われる場合の疫学的解明に役立つと考えられる。
