Risk factors for extended-spectrum β-lactamase-producing Serratia marcescens and Klebsiella pneumoniae acquisition in a neonatal intensive care unit

著者らは、イタリアのある大学病院の新生児集中治療室（NICU）におけるゲンタマイシン耐性基質特異性拡張型β-ラクタマーゼ（ESBL）産生Klebsiella pneumoniaeおよびSerratia marcescensの分子疫学、およびこれらの獲得に関連する危険因子について調査した。研究期間中（2004年4月から11月）、S. marcescensによる感染は6件、保菌は31件、K. pneumoniaeによる感染は6件、保菌は103件が認められた。両菌が同時に分離された新生児は24例であった。分子型別により、研究期間中に分離されたS. marcescens株およびK. pneumoniae株に対するパルスフィールド・ゲル電気泳動の単一の主要パターンを同定した。blaSHV-12、blaTEM-1およびaac(6’)-Ib遺伝子を含み、S. marcescensおよびK. pneumoniaeの両株から分離され、同一の制限酵素切断プロフィールを示す80 kbのプラスミドにより、第三世代セファロスポリンに対する耐性が、以前は感受性であった大腸菌に移行した。単変量解析により、出生時体重、在胎期間、および侵襲性デバイスの使用がS. marcescensおよびK. pneumoniae獲得と有意に関連し、一方、アンピシリンとゲンタマイシンによる経験的抗菌薬治療および入院期間のみが独立した危険因子であることが示された。結論として、プラスミド接合伝達、およびアンピシリンとゲンタマイシンによる経験的抗菌薬治療は、NICUにおけるゲンタマイシン耐性ESBL産生腸内細菌の選択および伝播に寄与した可能性がある。

監訳者コメント：
NICUにおける耐性菌の集団発生としてはMRSAが最も多いと思われるが、ESBL産生S. marcescensやK. pneumoniaeの集団発生も決して少なくない。本論文はその事例を分子疫学的手法も加えて詳細に分析している。データのしっかりした読み応えのある論文である。NICUの感染対策を担当する者は一読の価値がある。
