Nine-year downward trends in surgical site infection rate in southeast France (1995-2003)

本研究の目的は、フランス南東部の大規模な手術部位感染サーベイランスネットワークを用いて、1995年から2003年の手術部位感染の発生率の経時的傾向を推定することである。本ネットワークに2年以上参加した187の外科病棟のデータを解析した。手術部位感染率の経時的変化を、外科病棟内の患者集団の階層的ロジスティック回帰モデルを用いてモデル化した。対象患者200,207例中3,786例（1.9％）に手術部位感染が発生した。オッズ比0.95（95％信頼区間0.93～0.97）から推定した9年間の手術部位感染率の傾向は、1年あたり5％の手術部位感染率の低下と判定された。この低下は、研究期間を通して一定であり、ほぼすべての種類の外科手術（整形外科、消化器科、泌尿器科など）で認められた。全手術部位感染率は、9年間で45％低下した。この傾向は、外科病棟の多様性および患者の人口統計学的特性※の経時的な相違を考慮しても同様であった。このことから、1年あたり5％の低下は、手術部位感染を減少させるために外科病棟が実施した予防対策の結果と解釈するのが妥当なものであると考えられる。
