外科の主要4科における顔面へのはね返りリスクの多施設研究★★
Risk of facial splashes in four major surgical specialties in a multicentre study

本研究は、顔面の防護を行わない場合の曝露リスクの判定を目的とした、6カ所の医療施設でのフェイスシールドへの血液のはね返り汚染の分析である。血液のはね返り曝露を、全発生率、フェイスシールド上のはね返りの部位、外科の専門科、手術室スタッフに対するリスク、手術所要時間、および出血量に基づいて評価した。視覚的検査およびロイコマラカイトグリーンによる染色法による血液のはね返り汚染を、フェイスシールド600個で評価した。フェイスシールドを眼窩部(O領域)、眼窩周辺部(P領域)、およびマスク部(M領域)の3領域に区分した。視覚的検査では、フェイスシールドの50.5%(303/600)に血液のはね返り汚染が検出されたが、ロイコマラカイトグリーンによる染色法では66.0%(396/600)に血液汚染が検出された。O領域では36.6%(220/600)、P領域では37.8%(227/600)、M領域では57.0%(342/600)に血液汚染がみられた。手術室スタッフ別にみると、血液のはね返りの発生率は執刀医が83.5%(167/200)で最も高く、第一助手の68.5%(137/200)、手術室看護師の46.0%(92/200)がこれに続いた。専門科別では、心臓外科の発生率が75.3%(113/150)で最も高リスクであり、神経外科の69.3%(104/150)、消化器外科の60.0%(90/150)、整形外科の60.0%(90/150)がこれに続いた。

監訳者コメント:
HCVは結膜感染の事例もあるため、目の保護は職業感染予防策の観点から重要である。古典的な手法であるが、現実を知る上ではためになる論文である。
