Rapid control of an outbreak of vancomycin-resistant enterococci in a French university hospital

フランスの病院ではバンコマイシン耐性腸球菌（VRE）が認められる。著者らの病院でVREアウトブレイクが発生したため、VREの根絶に取り組むこととなった。アウトブレイク制圧のために、以下の介入を同時に実行した。（1）VRE制圧委員会の発足、（2）VRE保菌者の専用病棟への集団隔離、（3）接触患者の徹底的なスクリーニング、（4）直腸サンプルからVREを検出するための高感度技術の利用、（5）特定の抗菌薬の使用量減少を目的とした専任チームによる介入、（6）病院スタッフ全員に対する情報提供と教育、（7）VRE保菌者および接触患者の病院内の移動と再入院の電子的追跡。初発症例の入院後4週間で、vanA型バンコマイシン耐性Enterococcus faeciumの単一菌株の保菌者37例が病院の7部門で同定された。その後、再入院した接触患者1例がさらに同定された。VRE陽性患者39例中、2例が尿路感染症であり、37例に定着が認められた。便中のVRE濃度が測定可能であった32例中、23例は低濃度、9例は高濃度であった。低濃度の患者1例は、別の部門の伝播を引き起こした。今回の集中的、協調的、多面的戦略により、著者らの病院の広範囲にわたったVREアウトブレイクを終結させることができた。

アウトブレイクの制圧に必要なのは、気合と根性ではなくて、体系的で科学的なアプローチである。緊急事態にどのような体制を整えることができるか、病院の施設としての底力が問われる。
