A mobile laminar airflow unit to reduce air bacterial contamination at surgical area in a conventionally ventilated operating theatre

本研究の目的は、乱流型の換気を行っていた手術室の術野周囲の細菌汚染減少を図るための可動型層流装置の有効性を評価することである。76件の泌尿器科清潔開腹手術で細菌濃度の評価を行った。このうちの34件では可動型層流装置を併用した。各手術中に、静置培地を手術室の4カ所（患者エリア1カ所、周辺3カ所）に設置し、ニトロセルロース膜を手術器械台と創部近傍に設置した。4件の手術中に、0.5 μm以上の微粒子を検出するために空中浮遊微粒子の計測を行った。手術器械台上のニトロセルロース膜の平均細菌濃度は、標準的な換気条件下で2,730 cfu/m2/hであったが、層流装置を使用すると、超清浄手術室の推奨範囲内である平均305 cfu/m2/hまで有意に減少した（P＝0.0001）。創部近傍のニトロセルロース膜の細菌濃度は、層流装置なしで4,031 cfu/m2/h、層流装置ありで1,608 cfu/m2/hであった（P＝0.0001）。また、層流装置を使用すると微粒子数も減少した。手術室の4カ所でエアサンプリングを実施したところ、層流装置の有無別に有意差は認められなかった。可動型層流装置を乱流型の換気と併用することにより、高リスクの手術（例えば、人工物埋め込み）における術野周囲の細菌汚染を減少させることができる。

監訳者コメント：
確かに術野周囲の細菌汚染がコントロールできるようであるが、これが実際にインプラント手術の術後感染発生率を下げるかどうかについて、更なる検討が必要であろう。基礎データとしてはおもしろいと思う。
