Meticillin-resistant Staphylococcus aureus carriage among district nurse patients and
medical admissions in a UK district

限定した2つの地域集団におけるメチシリン耐性黄色ブドウ球菌（MRSA）保菌率を調査し、MRSA保菌に関連する危険因子を評価した。本研究は集団有病率調査としてデザインされ、ハンティングドンシャー地区の地域病院のmedical assessment unit※（MAU）の患者、および地区看護師※※が担当する患者（district nurse patient；DNP）集団を対象として実施された。全体で162名の参加者を登録し、内訳はMAUから91例、DNP集団から71例であった。MRSAの検出率はDNP集団21.1％［95％信頼区間（CI）11.6～30.4］、MAU集団6.6％（95％CI 1.5～11.7）であった。MRSA保菌と有意に関連することが明らかになった因子は、年齢（76.6歳、P＝0.008）、創傷または潰瘍の存在（P＝0.012）、過去1年以内の入院（P＝0.017）、MRSA感染/保菌歴（P＜0.001）、および過去6カ月以内の抗生物質使用（P＝0.016）であった。MRSA保菌の独立予測因子はMRSA感染/保菌歴（補正オッズ比8.53、95％CI 2.11～34.43、P＝0.003）のみであった。DNP集団は地域における重要なMRSAリザーバであり、この知見を高リスク患者のスクリーニングに関する政策に反映させる必要がある。

監訳者コメント：
MRSA感染/保菌の既往のある患者は、MRSA感染症に対する治療ないしは保菌に対する除菌治療を受けたかどうかにかかわらず、保菌に関するハイリスク群であると一般に考えられている。このスタディはその見解を後押しするものである。MRSA保菌者のスクリーニングによるあぶり出しが必要であるが、全員に対してスクリーニングを行うことが現実的ではない場合に、MRSA感染/保菌の既往のある患者を優先的にスクリーニングすべきであるといえる。
