The combined use of VIGI@ctR (bioMerieux) and fluorescent amplified length fragment polymorphisms in the investigation of potential outbreaks

優れたサーベイランスプログラムを用いても、病院感染を必ず認識できるとは限らず、そのためアウトブレイクに至る可能性がある。このリスクを低減するため、著者らは、リアルタイムの疫学情報システムVIGI@ctR（bioMerieux、Las Balmas、フランス）、および蛍光増幅断片長多型（f-AFLP）の微生物フィンガープリント法を利用した病原体の遺伝子に基づく関連性の迅速な確認または除外方法を併用した、病院感染の迅速検出モデルを提案する。このシステムによる1年間の調査結果を提示するが、ここでは合計306例が病院感染の疑い例と判定された。このうち281例（92％）が臨床所見によって「確定」し、16例（5％）は単なる保菌とされ、残りの9例（3％）は医師の協力が得られなかったために「無回答」と記録された。7件のアウトブレイクが疑われ、f-AFLP解析によってこのうち4件で分離株のクローンの関連性が確認された。すなわち、アウトブレイク1（緑膿菌の分離株4株）、アウトブレイク2（大腸菌の分離株3株）、アウトブレイク6（Candida parapsilosisの分離株2株）、アウトブレイク7（基質特異性拡張型βラクタマーゼ産生Klebsiella pneumoniae subs. pneumoniae 30株）である。これらの結果に基づき、著者らは、VIGI@ctRとf-AFLPの併用はグラム陽性菌、グラム陰性菌、酵母様真菌によるアウトブレイクの迅速な評価に有用であると結論づけた。

監訳者コメント：
効果的なアウトブレイク菌種ならびに菌株の選定は病院疫学解析の重要なスクリーニングのポイントである。本報告の取り組みに一定の効果のあることは評価されるが感度と特異度に関しては本取り組みだけでは何ともいえない。f-AFLP法は特許の関係でコストがそれなりにかかるのが難点でもある。
