手術部位感染（SSI）は、手術後の重要なアウトカム指標であり、スコットランドの全国的サーベイランスプログラムもこれを対象としている。退院後サーベイランスは、医療保障制度NHS（National Health Service）のプライマリケアおよび急性期医療の両方の領域におけるコストに重要な影響があるため、これらのデータを得るための確固とした方法を確立することが重要である。この研究では、退院後のSSI発生に関する情報を得るために、自動電話システムを使用した。対象は人工股関節置換術、人工膝関節置換術、および心臓手術の後に入院患者サーベイランスを実施した患者である。録音メッセージを使用して、創傷の状態を患者に質問した。これらの質問に対して、患者は電話機の番号を押すことによって回答した。研究対象集団は104例で、このうち18例の患者は自分が創傷感染を生じていると考えていたが、確認したところ退院後SSIに罹患していたのは9例のみであった。この自動電話システムに少なくとも1回の回答を行った患者は62例であった。今回のパイロット研究の対象は少数であったが、ある程度の改良を加えたうえで、退院後SSIサーベイランスのために使用を継続すべきであると思われた。

監訳者コメント：
手術後入院期間が比較的長いわが国におけるSSIサーベイランスでは、退院後サーベイランスを実施しなくても、より適切にSSI症例を見いだしているとも考えられているが、感染対策の観点からも医療安全の面からも医療経済の要求からも入院期間は短縮する傾向が強く、やはりわが国においても退院後サーベイランスがより重要になってくると予想される。自動電話システムによる人手を省略した調査は、サーベイランスに人的資源を投入しづらいわが国においても非常に参考になる。
