Predictive factors of meticillin resistance among patients with Staphylococcus aureus bloodstream infections at hospital admission

メチシリン耐性黄色ブドウ球菌（MRSA）は、スペインの医療施設、特に長期ケア施設で広く認められ、入院時のMRSA血流感染罹患率が増加している。本研究の目的は、黄色ブドウ球菌血流感染により入院を要する患者のメチシリン耐性の予測因子を明らかにすることである。1991年1月から2003年12月に、入院時の黄色ブドウ球菌患者を比較する症例対照研究を実施した。入院時のMRSA血流感染の症例患者（50例）を、入院時のメチシリン感受性黄色ブドウ球菌（MSSA）血流感染の対照患者（98例）と比較した。入院時のMRSA血流感染罹患率は、1991年の1,000入院あたり0.08例から2003年の0.37例へ有意に増加した（P＜0.001）。MRSA血流感染患者とMSSA血流感染患者を比較した単変量解析により、メチシリン耐性と、年齢＞60歳、女性、MRSA分離歴、および医療関連血流感染の間に有意な関連が認められた。糖尿病、血液透析、免疫抑制などの基礎状態、感染源、または死亡率は、両群で差は認められなかった。多変量解析により、MRSA血流感染の独立した危険因子として、MRSA分離歴［オッズ比（OR）41、95％信頼区間（CI）4～350］および長期ケア施設からの入院（OR 37、95％CI 4.5～316）が特定された。

市中感染によるMRSA血流感染患者が増加しているというスペインからの報告。日本でも小児の皮膚疾患など外来患者のMRSA感染が増加していると考えられており、世界的な懸案事項である。
