The place of guidelines in hospital-acquired pneumonia

最近の病院感染の増加により、病院感染肺炎が注目されるようになった。病院感染肺炎は死亡につながる病院感染としては最も頻度が高いものであり、全入院患者の約0.5～1％がこれに罹患する。病院感染肺炎によって医療関連合併症が有意に増加し、患者の入院期間が平均で最大13日間延長するため、病院の医療資源に大きな影響を及ぼす。昨年、British Society of Antimicrobial Chemotherapy※病院感染肺炎ガイドラインが学会のウェブサイトで公開され、予防、診断、治療の3本柱を対象としたエビデンスに基づく病院感染肺炎ガイドラインとしては、世界で2つのうちの1つとなった。本論文では病院感染肺炎ガイドラインの進歩と現状について、最近の発展、アプローチと転帰の相違、および新たな作業分野に注目しながら検討する。エビデンスに基づくガイドラインの適用により病院感染肺炎が減少すること、および患者の転帰が改善することが明示されている。
