Electronic surveillance of wall-mounted soap and alcohol gel dispensers in an intensive care unit

壁掛け式の石けんとアルコールゲルディスペンサーの押し下げを記録する、圧力測定抵抗器と小型制御装置からなる電池式記録装置について記述する。この装置には2秒間のロックアウトの設定がなされており、1回の記録は1回の手指消毒と一致するようになっている。記録装置は16床の集中治療室内の病床区域2カ所および入口にある壁掛け式ディスペンサーに設置した。英国保健省による救命救急医療最小データセットを使用した評価では、病床区域でのディスペンサー使用は、開放病床区域における患者の依存度スコアと相関（r）していた（r＝0.5、P＜0.01）。病床区域および入口のいずれのディスペンサーにおいても、その使用には1日24時間の中で大きな、しかし異なる変動があった。この記録装置は、石けんやゲルの使用をフィードバックすることによって、手指消毒の質の向上や教育のイニシアチブに役立つ可能性がある。

手指衛生の遵守を判定する方法は定まっていないが、実際の使用状況から検討するのが最も説得力があり、この論文にあるような“ちょっとした”テクノロジーを利用した記録方法は注目される。
