Nosocomial bacterial meningitis in adults: a prospective series of 50 cases

成人における院内髄膜炎の特徴を評価するために、院内細菌性髄膜炎50例の前向きコホート研究を実施した。これらの症例は16歳以上の患者に対する脳脊髄液培養により確認した。細菌性髄膜炎の典型的症状（頭痛、頸部硬直、および発熱）は症例の70％超で認められたが、意識障害を示したのは少数（41％）であった。脳神経手術歴、最近の頭部損傷、脳脊髄液漏出、または遠位の感染巣などの基礎疾患が症例の94％で認められた。脳神経手術歴のある患者で最も高頻度に認められた病原菌は黄色ブドウ球菌であり、32例中10例（31％）で認められた。耳炎、副鼻腔炎、または肺炎などの遠位の感染巣のある患者は、ない患者よりもStreptococcus pneumoniaeによる髄膜炎の罹患率が高かった［9例中6例（67％）対41例中7例（17％）、P＝0.01］。症例の50％で合併症が認められ、16％が死亡した。本研究は、市中細菌性髄膜炎患者と比較して、院内髄膜炎の成人は特定の病原細菌を有する異なる患者群であることを示している。基礎疾患、特に脳神経手術歴または遠位の感染巣が大多数の患者で認められ、死亡率および罹患率は高かった。
