Infection control practices in public dental care services: findings from one South African Province

南アフリカの1州において、歯科診療関連の血液媒介ウイルス伝播リスクの増加をもたらす感染制御対策について調査した。良好な感染予防・制御対策を実施するための適切な規定について、全24の州立歯科医院を調べ、感染予防・制御の知識を評価するために歯科医、看護師、および歯科助手を含む75名の職員に面接を行い、23種類の歯科処置を調べた。安全な感染制御対策に関する適切な規定が存在するにもかかわらず、知識と実践の間に乖離があることが、重要な知見であった。歯科処置中に保護眼鏡をしないこと、複数の患者への診察の間に手洗いをしないこと、消毒または滅菌前に器具を分解しないこと、および個々の患者に滅菌したドリルを使用しないことが明らかになった。不適切な感染予防・制御の指標として、潜血の迅速検出法を利用した。検査した患者への使用直前の歯科器具の25％、また歯科診療室および病棟の表面および周辺部位の37％で汚染が記録された。本研究の結論として、州立歯科医院では安全な歯科対策に関する規定があるにもかかわらず、診療においては知識が活用されていなかった。ヒト免疫不全ウイルス（HIV）の保有率が高い集団における、血液媒介ウイルス伝播リスクは無視しできない。

歯科診療料域における血液媒介感染予防策の徹底はどの国でも課題である。わが国においても、歯科領域の感染予防における医師・歯科衛生士・歯科技工士の教育・徹底が急務である。
