Monitoring the occurrence of wound infections after cardiac surgery

本研究の目的は、心臓手術後の手術創感染の発生状況をリアルタイムでモニタリングするために、画像技術を利用することである。この研究では、モニタリング中のケースミックス※の変動の影響を滞りなく算入できるリスク・スコアシステムを開発し、導入した。我々は、ロンドンの教育病院からルーチンに収集したデータを解析した。このデータは、2000年4月から2004年3月の間に心臓手術を受け、地域のサーベイランスプログラムの一環として手術創の追跡を行った2,146例の記録である。ロジスティック回帰分析を用い、アウトカム指標を退院前の手術創感染の診断としたリスクモデルを開発した。このモデルに含まれる因子は、手術創の数、患者の年齢、冠動脈バイパス手術と心臓弁置換術を組み合わせた手術、腎疾患、および入院から手術までの日数であった。このモデルは個々のデータセットに記録された転帰を十分に予測し（χ2＝3.81、P＝0.58）、著者らはこのモデルを、転帰をモニタリングするための画像ツールに導入した。このリスクモデルおよび関連する画像モニタリング法は、感染管理を支援するための有益なツールと考えられた。これをリアルタイムで用いることで、ケア中の問題点を速やかに特定することができ、適切なタイミングで是正措置をとることが可能となる。
