Reducing neonatal nosocomial bloodstream infections through participation in a national surveillance system

ドイツにおいて、新生児集中治療室（NICU）の極低出生体重患児を対象とした全国的院内感染サーベイランスシステム（NEO-KISS）が2000年に創設された。このプログラムには院内血流感染（BSI）と肺炎に焦点を当て、48カ所のNICUが参加した。このプログラムに3年以上参加したNICUのデータのみを採用し、年度毎の比較を行った。相対危険度とそれぞれの95％信頼区間（CI）を算出し、有意な危険因子を特定するためにロジスティック重回帰分析を行った。選択基準に合致した24カ所のNICUから、プログラム参加後3年間に、都合3,856例、152,437患者・日のデータを収集した。BSI発生率は、1年目の1,000患者・日当たり8.3例から3年目の6.4例へ有意に24％減少した。ロジスティック重回帰分析では、参加3年目のBSIは1年目より有意に減少した（オッズ比0.73、95％CI 0.60～0.89）。参加した初年度は、BSIの独立した危険因子であったが、肺炎では危険因子ではなかった。著者らのデータは、各集中治療室にデータのフィードバックを求める継続的なNICU院内感染サーベイランスへの参加により、BSI発生率が減少することを示唆している。

監訳者コメント：
データの集中と同時に、フィードバックを継続的に行うことで、血流感染症の発生率が減少することを示した論文である。肺炎のデータの改善が見られにくいのは、現状推奨されている予防策がまだ十分ではないことを示唆しているのではないだろうか？ わが国でも厚生労働省が行っている院内感染サーベイランス（JANIS）のNICU部門が設置されサーベイランス事業が展開されており注目される（http://www.nih-janis.jp/report/index.html）。
