Infection control practices and infectious complications in dermatological surgery

本研究の目的は、感染制御策の評価、および感染制御策が開業皮膚科医による皮膚科手術の感染性合併症に与える影響の評価である。Surgical Group of the Societe Francaise de Dermatologieに所属するボランティア73名が、3カ月間の前向き研究を実施した。良性および悪性のすべての腫瘍切除術のほか、研究期間中に実施した外科手術のデータを収集したが、皮脂嚢胞および膿皮症は除外した。合計3,491例の皮膚科手術を本調査の対象とした。術後感染は67例（1.9％）で発生し、表在性の化膿が手術部位感染の92.5％を占めた。発生率は、切開術単独の（1.6％）よりも再建術を伴う切開術後（4.3％）のほうが高かった。感染予防策は手術部位により様々であった。多変量解析により、いずれの種類の外科手術においても、出血性の合併症が感染の独立因子であることが示された。男性であること、免疫抑制療法、および滅菌手袋の非着用が、再建術を伴う切開術後の感染症の独立因子であった。すべての手術に病院の手術室の使用を必要とするわけではなかった。再建術を伴う切開術、または鼻など特定の解剖学的部位の切開術においては、感染予防策をより重視すべきことは明らかである。これらの手術の最適な指針を確立するために、さらなる研究が必要である。

監訳者コメント：
外来ベースの、しかも清潔操作が可能な皮膚科的な外科処置に伴う創感染率に関する報告であり、今後わが国でも外来医療における感染制御の重要性が増す中、患者に対してのインフォームド・コンセントの資料として一つのベンチマークとなろう。
