Successful decolonization of meticillin-resistant Staphylococcus aureus in paediatric patients with cystic fibrosis (CF) using a three-step protocol

メチシリン耐性黄色ブドウ球菌（MRSA）は、嚢胞性線維症（CF）の細菌性病原体であることが知られているが、その臨床的影響は多様である。本研究の目的は、MRSAに対する三段階の除菌プロトコール（Belfast CF MRSA除菌プロトコール）の有効性を評価することである。5年の研究期間中に治療を受けた患児17例中8例（47％）は、リファンピシンおよびフシジン酸の5日間経口投与1コースで除菌に成功した。1コース目の治療終了時に培養陽性であった11例に対する2コース目の5日間経口投与後には、成功率は12例（71％）に増加した。さらに4例で静脈内テイコプラニン投与により除菌を達成し、除菌率は17例中16例（94％）に増加した。これらの成績は、報告されている他の試験と比べても遜色がなく、嚢胞性線維症患児のMRSA除菌が高い確率で成功しうることを示している。

嚢胞性線維症（cystic fibrosis）は白色人種に多い先天性疾患であり、膵外分泌や小腸機能低下、発汗異常、泌尿生殖器系異常を伴うが、最も特徴的な症状は気管支拡張症を伴う慢性気道感染症である。ブドウ球菌属菌（Staphylococcus spp.）や緑膿菌（Pseudomonas aeruginosa）が慢性的に下気道に常在することから、耐性菌の出現がしばしば問題となる。この疾患においては耐性菌による感染症が繰り返されることから除菌を検討することとなるが、その他の疾患においては一般的に常在菌を除菌する必要はないので、安易な類推による適応拡大は許されないことを強調したい。
