Changes in the epidemiology of meticillin-resistant Staphylococcus aureus associated with the emergence of EMRSA-16 at a university hospital

本研究では、University Hospital of the Canary Islandsにおけるメチシリン耐性黄色ブドウ球菌（MRSA）の分子疫学を調べ、6年間にわたる疫学の変化を評価した。2000年5月から2003年12月に臨床データおよび疫学的データを収集し、分離株にパルスフィールド・ゲル電気泳動法（PFGE）、MLST（multi-locus sequence typing）、SCCmecタイピング、およびspaタイピングを実施した。2000年以降、当病院のMRSA感染率が増加しているが、これと同時にEMRSA-16クローン（ST36-MRSA-II）の出現と伝播、およびイベリアのクローン（ST247-MRSA-I）からの置換が生じている。遺伝子型の変化は、疫学的プロファイルの変化と関連していた。患者の平均年齢および60歳を超える患者の割合（P＝0.01）、および呼吸器感染率は有意に増加した（P＝0.001）。EMRSA-16出現後にMRSA分離株のゲンタマイシンおよびテトラサイクリン感受性は増加した（P＜0.001）。PFGE、SCCmec、およびMLSTを同時に調べることは、これらの変化を理解すること、および当病院の患者集団で伝播しているクローンを明らかにすることに有用であった。

EMRSAは英国におけるMRSA流行株のことであり、発見された順番に序数が付けられている。分子疫学的解析のマーカーには色々な方法があり、いずれも異なる遺伝子変異の特徴を捉えているためこれらを組み合わせて多様性を評価する試みが進んでいる。相互の多型性マーカーの変動をどのように評価すべきかにつては今後の知見が必要である。
