Postoperative infections after major heart surgery and prevention of ventilator-associated pneumonia: a one-day European prevalence study (ESGNI-008)

主要な心臓手術を受けた患者の術後感染率に関するデータは、ほとんど発表されていない。術後感染予防のための標準的対策が、どの程度遵守されているかも明らかではない。本研究では、欧州13カ国の42の医療機関で心臓手術を受けた患者における感染率、特に人工呼吸器関連肺炎の罹患率を評価した。調査当日の心臓手術後の患者は321例であり、そのうち164例（51％）が人工呼吸器で管理されていた。全体の感染率は26.8％であった。調査当日にみられた感染症全体のうち57％が下気道感染であった。他の感染症は、静脈留置カテーテル関連血流感染（2.8％）、手術部位感染（2.2％）、尿路感染（0.9％）、および術後縦隔炎（0.9％）などであった。人工呼吸器を使用していた患者のうち55例（33.5％）は半臥位で介護されておらず、36例（22％）は抗菌フィルターを装備していない熱・湿気交換器※を使用し、78例（47.6％）ではカフ内圧がモニターされていなかった。持続的声門下吸引を行っていた患者は13例（8％）のみで、64例（39％）では定期的体位変換（postural oscillation）を実施しておらず、23例（14％）で胃過膨満に対する積極的な予防が実施されていなかった。結論として、欧州各地の集中治療室から得られたこれらのデータは、重要な患者集団における術後感染に関する情報を提供し、心臓手術を受けた患者の人工呼吸器関連肺炎の予防を目的とした積極的介入の必要性を強調するものである。
