Protecting staff against airborne viral particles: in vivo efficiency of laser masks

レーザー手術用マスク（laser mask）がレーザー手術中のウイルス粒子吸入の予防に用いられている。手術用マスクとレーザー手術用マスクの空気中塵埃粒子の濾過機能をFFP2規格※に準拠した呼吸器防護具と比較するため、8名のボランティアでクロスオーバー試験を行った。手術用マスクとレーザー手術用マスクは、通常の方法で装着した場合とテープで顔に接着した場合を調査した。粒子の平均減少数は、テープ非接着時の手術用マスク3.0倍［95％信頼区間（CI）1.8～4.2］、テープ非接着時のレーザー手術用マスク3.8倍（95％CI 2.9～4.6）、テープ接着時の手術用マスク7.5倍（95％CI 6.5～8.5）、テープ接着時のレーザー手術用マスク15.6倍（95％CI 13.5～17.8）、FFP2半面体呼吸器防護具102.6倍（95％CI 41.2～164.1）であった。レーザー手術用マスクの防護率はFFP2呼吸器防護具より有意に低く（P＝0.02）、手術用マスクよりごくわずかに高いだけであった。レーザー手術用マスクを呼吸器防護のために継続的に使用することには問題がある。マスクをテープで顔に接着することでは、防護率はわずかしか改善しなかった。

監訳者コメント：
CDCの環境のガイドラインではサージカルスモークの危険性を指摘している。サージカルスモーク中にはパピローマウイルスなどが混入することが指摘されており、組織焼灼による燻煙を呼吸器曝露することは発癌性の観点も含めて避けるべきである。本論文は呼吸器防護具の防護性能をサージカルスモークに関して裏付けるものである。
