医原性のプリオン伝播の予防のための外科用鋼製器具の蛋白質汚染の評価における承認されたニンヒドリン試験およびビウレット試験の感度★

病院の滅菌サービス部門(sterile service department;SSD)の蛋白性物質、特にプリオンの除去における洗浄消毒器サイクルの有効性を確認するために、ガイドラインではニンヒドリン試験またはビウレット試験などの生化学試験をルーチンに行うことが推奨されている。これらの方法の有効性は、機器の効果的なサンプリングおよび用いる試験の感度の両方に依存する。2種類の市販の汚染評価試験の感度を、外科手術用鋼製器具表面のME7脳ホモジネートを対象として評価した。対照として、医療機器上の低レベルの汚染を迅速に視覚化・評価できることがすでに示されている落射型微分干渉/エピ蛍光(episcopic differential interference contrast/Epi-fluorecence;EDIC/EF)顕微鏡を、高感度の蛍光試薬SYPRO Rubyと組み合わせて視覚化する方法を行った。ニンヒドリン試験では、ボランティア75%の最小検出レベル(MLD75)は9.25 μg[95%信頼区間(CI)8.6~10.0 μg]であった。ビウレット試験のほうが高感度で、MLD75は6.7 μg(95%CI 5.4~8.2 μg)であった。しかし、EDIC/EF顕微鏡法では、はるかに低濃度の蛋白質汚染(pg単位)を視覚化することができた。今回の知見から、これらの2種類の承認された洗浄度に対する比色試験の感度は比較的低いことが明らかである。本研究は、多量(最大6.5 μg)の脳由来の蛋白質汚染が未検出であった可能性があり、これらの試験方法では機器は清浄と見なされていたことを示している。神経外科用機器などの「高リスク」機器でのプリオン病の医原性伝播リスクを低下させるために、より高感度の清浄度評価法を適用するべきである。

監訳者コメント:
プリオンの完全な不活化は極めて困難であり、高額で繰り返し使用せざるを得ない器具の除染方法が課題である。適切な洗浄処理により物理的のプリオンコピー数を減少させることも重要なポイントである。このため、洗浄後の評価に適切な指標を置くことが求められる。さらなる検討が必要であろう。
