高齢者におけるメチシリン耐性黄色ブドウ球菌(MRSA)の保有率は高率であるにもかかわらず、この患者集団の院内MRSA血流感染の転帰は十分に調査されていない。本稿では症例対照研究を実施して、65歳以上の患者(症例群)と65歳未満の患者(対照群)の病院内MRSA血流感染の転帰を比較した。430床の第三次教育病院で、18歳以上の黄色ブドウ球菌血流感染入院患者100例を研究対象とした。測定項目は、共存疾患、来院時の疾患の重症度、抗菌薬治療、血行性合併症、および死亡率とした。全死亡率は症例群のほうが対照群よりも有意に高かった[36%対12%;オッズ比(OR)4.1、95%信頼区間(CI)1.4~14、P<0.01]。肺感染症は高齢患者群のほう若年対照群より高頻度に認められた(34%対16%;OR 2.7、95%CI 1.1~8.1、P=0.04)。ロジスティック回帰分析により、以下の変数が高齢患者のMRSA血流感染と独立した関連因子として認められた。内科病棟への入院(OR 3.1、95%CI 1.3~7.6、P=0.02)、中心静脈カテーテル以外に由来する血流感染(OR 3、95%CI 1.2~7.6、P=0.02)、および死亡(OR 3.7、95%CI 1.3~11、P=0.02)であった。バンコマイシン投与を受けた患者では、症例群のほうが対照群と比べて腎機能低下のためにバンコマイシンを減量して治療された患者が多かった(64%対31%;OR 4、95%CI 2~9、P=0.01)。これらのデータは、MRSA血流感染は、高齢患者の非常に高い死亡率と関連していることを示唆している。これらの患者集団のMRSA獲得を予防することが極めて重要である。

監訳者コメント:
高齢者におけるMRSA血流感染症は、若年者と比較すると、カテーテル関連が少なく、医療関連肺炎との関連が強い傾向が認められた。いずれにせよ、MRSA感染対策が重要であることに変わりはない。
