Reduction of surgical site infections after Caesarean delivery using surveillance

1997年以降、Krankenhaus Infektions Surveillance System※（KISS）では、帝王切開後の手術部位感染についてのデータを収集している。本研究の目的は、医療関連感染のサーベイランスおよびフィードバックにより帝王切開分娩後の感染率を減少させることができるかどうかを判定することであった。3年間以上KISSに参加した部門のみを本解析の対象とした。KISS参加1年目、2年目、および3年目の帝王切開感染率で有意差を比較した。KISS参加1年目および3年目の相対リスクを算出した。帝王切開後の手術部位感染の有意な危険因子（リスクファクター）を検出するため、参加3年目をパラメータの1つとした多変量ロジスティック回帰分析を実施した。52の産科婦人科のうちの26部門が、本研究の組み入れ基準を満たしていた。これらの26部門で17,405件の帝王切開を実施し、331例の手術部位感染を記録した（1.9％）。帝王切開後の手術部位感染率は、KISS参加3年目（1.6％）はKISS参加1年目（2.4％）と比較して有意に減少し、相対リスクは0.63［95％信頼区間（CI）0.48～0.82］であった。ロジスティック回帰分析により、3年間にわたるKISS参加は手術部位感染率減少の独立因子であることが確認された（オッズ比0.64、95％CI 0.49～0.83）。他のタイプの医療関連感染で以前に示されたように、本研究では、継続的なサーベイランスおよび層別化した参照データの比較により、帝王切開後の手術部位感染率を減少させ得ることが実証された。
