Bacillus spp. among hospitalized patients with haematological malignancies: clinical features, epidemics and outcomes

2000年4月から2005年5月に、著者らの血液科で治療を受けた患者で350件の菌血症エピソードが発生した。これらのエピソードのうち228件はグラム陽性菌が起因菌であり、コアグラーゼ陰性ブドウ球菌および黄色ブドウ球菌が最も多かった。122件はグラム陰性菌が起因菌で、大腸菌、Acinetobacter baumannii、および緑膿菌が優勢であった。Bacillus属による菌血症は、これらのエピソードのうち12例の患者に認められた12件であり、全菌血症エピソードの3.4％を占めた。検査した12菌株のうち、Bacillus licheniformisが7株、Bacillus cereusが3株、Bacillus pumilusが2株であった。菌血症エピソードのうち血流感染が7件、肺炎が3件、重度の腹痛および肝機能障害が1件、カテーテル由来血流感染が1件であった。B. licheniformisが、同時期に入院していた患者5例から分離された。この集団発生は皮膚消毒時に使用した非滅菌脱脂綿と関連があった。B. cereusおよびB. licheniformisの分離菌株はセフェピム、カルバペネム系、アミノグリコシド系、およびバンコマイシンに感受性があったが、B. pumilus分離菌株はキノロン系およびバンコマイシン以外の全抗生物質に耐性があった。2例の死亡が確認された。結論として、Bacillus属は血液腫瘍患者に重篤な感染症を引き起こし、診断および治療上のジレンマをもたらし、また高罹患率および死亡率の原因となりうる。B. cereusおよびB. licheniformisはいずれも、好中球減少患者で重篤な感染症を引き起こす“新しい”グラム陽性菌である可能性がある。
