2003年の10カ月間に、スウェーデンの郡立病院の異なる病棟の15部門で治療を受けた患者に由来するルーチンの連続臨床培養で、メチシリン耐性コアグラーゼ陰性ブドウ球菌のスクリーニングを行った。分離菌株間の遺伝的相同性をパルスフィールドゲル電気泳動法(PFGE)で分析した。この結果を、第三次紹介病院で過去に分離された多剤耐性表皮ブドウ球菌(Staphylococcus epidermidis)と比較した。合計428のコアグラーゼ陰性ブドウ球菌の分離菌株を特定し、そのうち188株(44%)がメチシリン耐性であった。S. epidermidisの3つのクラスター(グループA、B、およびC)はそれぞれ10を超える分離菌株からなり、PFGEによるDNA相同性は90%を超えていた。グループA(15株)とB(13株)の菌株はおおむねゲンタマイシン、ST合剤、およびクリンダマイシンに耐性があり、24例の患者に由来し、そのうちの21例は検体採取前に集中治療室(ICU)で治療を受けていた。3番目のクラスターであるグループCは、患者14例に由来する14の分離菌株からなっていた。これらの患者のうちICUに入室したのは2例だけであり、いずれも1日未満であった。グループCの分離菌株は、グループAとBの分離菌株よりも耐性が低かった。グループAに属する分離菌株は、紹介病院で患者から分離した多剤耐性S. epidermidisとPFGEプロフィールが同じであった。本研究は、郡立病院内の特にICUでメチシリン耐性コアグラーゼ陰性ブドウ球菌クローンが存続および拡散していること、および多剤耐性クローンが郡立病院と紹介病院との間で拡散した可能性を示している。
