A napkin-associated outbreak of Burkholderia cenocepacia bacteraemia in haemodialysis patients

本稿では、イタリア、ベロナの2カ所の血液透析施設におけるカテーテル関連菌血症38例の集団発生について報告する。ヒトの血液、中心静脈カテーテルを取り扱うために使用される市販の滅菌包帯キットに含まれていた単品個別包装の消毒ナプキンから、バークホルデリア・セパシア（Burkholderia cepacia）グループ菌株が分離された。血液培養およびナプキンから分離した微生物を標準的な方法によって同定したところ、分子解析によりバークホルデリア・セノセパシア（Burkholderia cenocepacia）（遺伝子型III）であることを確認した。パルスフィールドゲル電気泳動解析では、臨床分離株はナプキンから分離したB. cenocepaciaと識別不能、あるいは近縁であった。結論として、本研究により、第4級アンモニウムに浸漬された市販のナプキンが、品質が保証されていたにもかかわらず、汚染されて感染源となったことが判明した。

監訳者コメント：
バークホルデリア・セパシア（Burkholderia cepacia）グループ菌は、水周りを中心とした環境で生き延びるブドウ糖非発酵グラム陰性桿菌である。ときに消毒薬耐性が問題となることがあるので注意が必要である。アウトブレイク対応における環境調査の実際としても一読に値する報告である。
