手術部位感染のサーベイランスは、重要な感染制御対策の1つである。Scottish Surveillance of Healthcare Associated Infection Programmeの一環として帝王切開術を選択し、手術部位感染の発生をモニターして報告することとした。2002年の後半と2003年の第1四半期の35週間に帝王切開術を受けた患者715例について、データを前向きに収集した。このうち80例（11.2％）が手術部位感染を発現し、その57例（71％）は退院後のサーベイランスで検出された。感染に関連する危険因子（リスクファクター）を分析した。手術部位の縫合にステープルよりも皮内縫合を選択することが、有意に低い感染率と関連した（P＝0.021）。肥満女性は、体格指数が正常の女性より有意に多数の感染を発現した（P＝0.028）。サーベイランスの結果が広く公表されたことで、医師らは、この患者集団の手術部位感染に対する体格指数および皮膚縫合の選択の影響を認識するようになっている。これらのデータの解析により、地域診療が見直されつつある。また、これらの結果は、この患者集団で手術部位感染が検出された場合は、退院後のサーベイランスが重要であることも示している。継続的なデータ収集と時宜を得た結果の公表は、診療方法の見直しの触媒として作用する重要な要素である。

“手術部位の縫合にステープルよりも皮内縫合を選択することが、有意に低い感染率と関連した（P＝0.021）”とある。縫合糸は、使用した素材（製品）の影響などを大きく受けるため、こうした詳細な解析に基づく評価が求められる。
