Impact of safety needle devices on occupationally acquired needlestick injuries: a four-year prospective study

複数の安全な皮下注射針装置の導入が針刺し・切創報告件数に及ぼす影響を評価するために、4年間にわたる前向き研究を国民保健サービス・バーミンガム大学病院（University Hospital Birmingham National Health Service Foundation Trust）で行った。4カ所の臨床区域の針刺し・切創報告件数に関する2001年以降のデータである。2002年の鋭利物に対する認識強化戦略の後、針刺し・切創報告件数は2001年の100,000器材あたり16.9件から100,000器材あたり13.9件に減少した（P＝0.813）。標準的な研修のみを実施した2003年には、針刺し・切創報告件数は100,000器材あたり20件に増加した。しかし、その後の3種類の安全装置付き針の導入および研修の同時開催により、2004年に針刺し・切創報告件数は有意に低下し、100,000装置あたり6件になった（P＝0.045）。使用者の安全針に対する満足度と支持も非常に良好であった。これらの結果は、安全装置付き針が臨床現場に導入され、適切な研修が行われると、職業上の針刺し・切創件数が顕著に低下する可能性があることを示唆している。

監訳者コメント：
標準的な研修に加えて誤刺防止機構付き安全器材を導入したことから、医療従事者の針刺し・切創を減少させたとする報告である。針刺し・切創は受傷者の自発的報告で評価せざるを得ないという限界はあるが、使用器材あたりの報告件数として有意の効果を認めたことに注目すべきである。
