Quantitative analysis of residual protein contamination on reprocessed surgical instruments

盲検試験により、手術器具トレーにある通常の洗浄および滅菌後の「使用可能」器具について調査を行った。これらの再利用される器具は、イングランドとウェールズの5カ所の全国保健サービス（National Health Service）トラストの病院の滅菌部門からのものであった。残留蛋白質およびペプチド汚染の測定は、酸ストリッピング（acid stripping）、構成アミノ酸の加水分解、および定量的総アミノ酸分析により実施した。120の器具を分析し、個々のトレーにおける、器具あたりの残留蛋白質汚染濃度の中央値は267、260、163、456、756 μgであった。器具の走査型電子顕微鏡検査とエネルギー分散型X線分光分析により、組織沈着物は表面上に限局することが示されたが、全蛋白質汚染と器具の複雑さとの間に有意な相関はなかった。残留蛋白質濃度の最大値は、扁桃摘出術およびアデノイド手術に使用する器具で認められた。

監訳者コメント：
手術器具の洗浄滅菌後の汚染がかなりの程度発生していることを明らかにした本論文の意義は大きい。本号に他１編を見るこの問題は、手術器具の洗浄滅菌工程の見直しの必要性を示唆しているのかもしれない。
