Potential compromise of hospital hygiene by clinical waste carts

大容量の廃棄物保管カートは病院では一般的に使われており、日常の医療廃棄物管理に役立っていることは確かである。カートは一次医療廃棄物容器の移送および一時保管用に使用され、そうした廃棄物を臨床部門から受け取るために、病院建物の近くまたは内部に置かれることが多い。大ロンドン地域全域の9カ所の急性期病院から無作為に選択した大容量医療廃棄物カートの調査により、23カートすべての外面が汚染されていることが明らかになった。13カート中8カートは内部表面も汚染されており、5カートの底面では血液付着と遊離液体の痕跡を認めた。黄色ブドウ球菌と腸球菌がカートの蓋（7例）と車輪（10例）から少数採取され、大腸菌、エンテロバクター属、および緑膿菌が別の5カートの車輪から採取された。2カートはアスペルギルス属に重度に汚染されていた。医療廃棄物由来の病原体が、汚染された大容量廃棄物カートから、より広範な病院環境へと伝播する可能性がある。大容量カートを臨床区域外で、可能であればあらゆる病院建物の外側で管理するのが適切なようである。このことは、受託業者の供給するカートが単一の病院専用または全国保健サービストラスト（National Health Service Trust）専用ではない場合、特に重要になる。
