Infections following major heart surgery in European intensive care units: there is room for improvement (ESGNI 007 Study)

心臓の大手術を受ける患者では、院内感染のリスクが増加する可能性がある。欧州の集中治療室（ICU）における心臓大手術患者の感染の発生率とタイプ、およびそのケアの質を評価するため、欧州の心臓大手術ICUから選択して質問票を郵送した。欧州7カ国の17病院が参加した。全体で、対象ICUの53％が心臓大手術患者のみを受け入れ、残る47％はその他の患者も受け入れていた。研究期間中、患者11,915例が心臓大手術を受け、1,181例（9.9％）に1つ以上の院内感染が発生した。人工呼吸器関連肺炎が最も頻度の高い感染で（中央値3.8％、四分位範囲［IQR］1.8～4.9）、次いで手術創感染（中央値1.6％、IQR 0.8～2.3）、カテーテル関連血流感染（中央値1.3％、IQR 0.8～2.1）、縦隔炎（中央値1.1％、IQR 0.4～1.6）、尿路感染（中央値0.6、IQR 0.4～1.4）、院内感染による心内膜炎（中央値0.2％、IQR 0.0～0.9）であった。死亡率の中央値は4.7％（IQR 2.7～8.4）、感染関連死亡率の中央値は1％（IQR 0.5～2.7）であった。人工呼吸器関連肺炎に関しては、18％のICUでは、診断をルーチンに実施していなかった。微生物に関する情報は、症例の35％が定量的情報であり、65％が定性的情報のみであった。感染症専門医が人工呼吸器関連肺炎管理に定期的にかかわっていたICUはわずか35％で、人工呼吸器関連肺炎の治療で抗菌薬のde-escalation※を実施していたICUは、全体の59％であった。欧州の心臓大手術ICUでは、依然として術後感染率が高い。人工呼吸器関連肺炎の診断および治療に対する、よく知られているルーチンの診療が、多くの欧州の施設では定期的に実施されていない。

監訳者コメント：
Clean surgeryに分類される心臓外科手術では、そもそもSSIが上位には上がってこない。人工呼吸器関連肺炎が最大の問題であるにもかかわらず、感染症や呼吸器の専門医が関与している施設の割合が低いことが、mortalityの低下の重要な一因となっている。今後の課題を提起する社会的意義をもった調査報告である。
