Results from a four-year study on the prevalence of nosocomial infections in Franche-Comte: attempt to rank the risk of nosocomial infection

本研究の目的は、2001～2004年にフランシュ・コンテ地方で年1回実施した罹患率調査の結果を分析することによって、院内感染リスクを患者のタイプ別に順位づけすることである。患者（14,905例）を内因性危険因子（年齢、免疫抑制、MacCabeスコア）の数に応じて4つのカテゴリに分類した。感染の全罹患率は6.1％で、患者カテゴリによって1.93％（危険因子なし）から15.2％（危険因子が3つ）までばらつきがあった。侵襲的手技に関連する院内感染発生率は30.9％、多剤耐性菌の交差汚染に関連する院内感染発生率は12.3％であった。これらの割合は、患者タイプとは無関係であった。院内感染罹患率は、2つまたは3つの危険因子をもつ患者では経時的に減少したが、危険因子のない患者では一定であった。院内感染のうち40％を超えるものは（侵襲的手技および多剤耐性菌による交差汚染に関連）患者カテゴリにかかわりなく、予防が可能であった。本研究は、少なくとも30％の院内感染が予防可能であったことを示唆している。
