韓国の大学病院における接合性プラスミド由来DHA-1型β-ラクタマーゼ産生大腸菌分離株の出現

誘導性の表現型としてcefoxitin耐性を有する大腸菌分離株7株が、韓国の病院で2002年11月から2003年7月に検出された。受容菌としてアジ化ナトリウム耐性大腸菌J53を用いて、フィルター接合法(filter mating method)により接合性の試験を行った。すべての分離株およびその接合菌に対し、微量液体希釈法による最小発育阻止濃度測定、等電点電気泳動、SHV、TEM、CTX-M、およびDHA型β-ラクタマーゼのPCR、およびDNAシークエンスを行った。分離株7株の制限酵素XbaI消化によるゲノムDNAのバンドをパルスフィールドゲル電気泳動(PFGE)で分離した。等電点電気泳動、PCR、およびシークエンスにより、分離株はすべてblaTEM-1-likeおよびblaDHA-1遺伝子を有することが明らかになった。2株はblaCTX-M-14遺伝子も有していた。接合実験による耐性の伝達が7分離株すべてで成功したことから、大腸菌分離株のblaDHA-1を含むプラスミドは自己伝達が可能であることが示唆される。分離株は病棟または集中治療室の患者から採取され、全分離株はDHA-1産生株の分離前にβ-ラクタム系抗菌薬に曝露されていた。分離株7株のPFGEによるパターンは5つであることが示された。誘導性のβ-ラクタム系抗菌薬耐性表現型を有する大腸菌の散発的な感染が認められた。接合性プラスミドがコードするDHA-1は耐性表現型を有していた。DHA-1産生菌の伝播は、クローンの伝播と耐性遺伝子の水平伝達によるものであった。
