Nosocomial and community-acquired meticillin-resistant Staphylococcus aureus infections in hospitalized patients (Spain, 1993窶・003)

メチシリン耐性黄色ブドウ球菌（MRSA）の割合の臨床的・人口統計学的特徴、トレンド、および地理的なばらつきを明らかにするため、スペインの入院患者における感染流行に関する一連の年次調査を1993～2003年に実施した。試験期間中、スペインの17地域の296救急病院の患者で合計8,312例の黄色ブドウ球菌感染が認められた。全体で、23.8％の黄色ブドウ球菌がメチシリン耐性と報告された。MRSAの割合は地域により、また試験期間中、大きなばらつきがあった。MRSA罹患率は院内感染患者（31％）で市中感染患者（14％）より2倍高かった（P＜0.001）。一方11年間を通して、MRSA分離株の割合は、院内感染患者（22％から41％へ；P＜0.001）および市中感染患者（7％から28％へ；P＜0.001）で増加傾向にあった。最近の3年間（2001～2003年）の地理的なばらつきは求心性の勾配を示し、MRSA罹患率はスペイン南西部で最も低く、中心部で最も高かった。地域間でMRSAの割合に5倍近い差がみられた（範囲10.3～54.5％）。血流感染と比較して、それ以外の部位での感染はMRSAが原因である可能性が高かった（手術部位、尿路、皮膚、呼吸器の感染の補正オッズ比は、それぞれ1.2、1.2、1.5、2.1）。

監訳者コメント：
他国のMRSA罹患率に関する状況を日本と比較することは興味深い。ヨーロッパではオランダや北欧諸国のようにMRSAの割合を低く保つことに成功している国がある一方で、割合がどんどん上昇している国もある。スペインは後者の中の一国であり、その割合は米国や日本ほど高くはないものの、同国の状況に関する危機感が本文献から読み取れる。
