Association of broad-spectrum antibiotic use with faecal carriage of oxyiminocephalosporin-resistant enterobacteriaceae in an intensive care unit

抗生物質投与と糞便からの第三世代セファロスポリン耐性腸内細菌科細菌の検出との関連について、熱傷集中治療室の患者を対象に研究を行った。これらの分離株中の基質拡張型βラクタマーゼ産生菌の存在についても調べた。
　1998年1月から1999年6月までに集中治療室に入室した患者72例中43例から、2個以上の直腸スワブ試料を採取した。分離されたすべての腸内細菌科細菌について、米国臨床検査標準化委員会（National Committee for Clinical Laboratory Standards；NCCLS※）の方法を用いて抗生物質耐性検査を実施した。抗生物質治療を受けた患者30例中、糞便中に第三世代セファロスポリン耐性腸内細菌科細菌が認められたのは10例のみであった。Fisherの直接確率検定により、オキシイミノセファロスポリン（第三世代セファロスポリン系）（P＝0.002）またはカルバペネム系（P＝0.003）の投与と、糞便からの第三世代セファロスポリン耐性腸内細菌科細菌の分離との関連が示された。オキシイミノセファロスポリンの投与は、糞便中微生物叢の耐性株選択をもたらした。
