Risk of conjunctival contamination in total joint arthroplasty

本研究は人工関節全置換術における結膜汚染による感染伝播への関心から開始した。29の人工股関節全置換術および26の人工膝関節全置換術で使用された顔面および眼用の個人防護具（Sterishield）110セットの肉眼的に確認できる汚染を前向き解析として調査した。さらに、眼汚染のリスク知覚および人工膝関節全置換術および人工股関節全置換術の術中の現行の眼用防護具に関する質問票を英国整形外科学会（British Orthopaedic Association）の会員1,500名に郵送した。すべてのSterishieldに平均で203の血液および脂肪のスポットからなる肉眼的汚染が認められた。汚染スポット数はSterishieldの下半分で有意に大きかった。834の有効回答のうち、回答者の48％が、適切な眼用防護具をルーチンで使用していなかった。28％が眼用保護具は必要ないと感じていた一方、45％は利用可能な眼用防護具は視界を損なうと感じて使用していなかった。回答者107名（13％）が眼の汚染を認識したことを報告しており、さらに回答者21名（2.5％）は汚染後の結膜感染に少なくとも1回は気づいていた。本研究では、股関節や膝関節の人工関節全置換術は、脂肪および血液による結膜汚染のリスクが高いことを示している。英国における適切な眼および顔面の防護具使用に関する報告では使用率が低いことが示された（52％）。
