A practical tool to assess the incidence of nosocomial infection in Japanese intensive care units: the Japanese Nosocomial Infection Surveillance System

感染制御専門家は病院内の感染率を算出する手段はあるが、これらを評価する手段がない。このため、観察された感染率と外部標準との比較をするための実用的手段が必要である。日本の院内感染対策サーベイランス（JANIS）システムの集中治療室部門から得られたデータを基に、リスク補正した指標として標準化感染率を算出するための表計算プログラムを開発した。多変量解析で院内感染の発症に関連することが示された因子から、層別化のためにAPACHE IIスコア（0～10、11～12、21以上）、手術（あり、なし）、および人工呼吸器の使用（使用者、非使用者）という3つの因子を選択した。2001年の感染率を算出して（1,000患者・日あたり）、3つの因子ごとに層別化し、ベンチマークとした。日本の感染率ベンチマークに基づいて標準化感染率を算出できるように、Microsoft Excelソフトウェアで表計算プログラムを設計した。表計算プログラムの使用者は、観察された院内感染数および患者・日の数値をAPACHE IIスコア、手術の有無、および人工呼吸器の使用ごとに入力することになる。日本の8カ所の集中治療室に適用したところ、この表計算プログラムは、各集中治療室の院内感染発生率には相対的な相違および経時的変化がみられることが明らかになった。
