Propagation of methicillin-resistant Staphylococcus aureus due to the overloading of medical nurses in intensive care units

病院感染の罹患率および死亡率は増加を続けている。感染制御の有効性は、費用削減に寄与するだけでなく、病院における医療の質の重要な指標となっている。今回の3年間の前向き研究の目的は、看護師の日常的労働負荷を調査し、外科集中治療室におけるメチシリン耐性黄色ブドウ球菌（MRSA）感染の拡大と相関があるかを、治療スコアリングシステム（Therapeutic Intervention Scoring System；TISS）を用いて調べることである。対象とした外科集中治療室は継続教育に強い配慮を行っているが、MRSA発生率は依然として高い。TISSによって、外科集中治療室の看護師は平均57％の過重労働であると推定された。MRSA感染は、看護師の過重労働が平均25％を超えていた期間中に発生した。研究期間中、47例のMRSA感染が新たに検出された。
これらの結果から、特に外科集中治療室の看護職員について、適切な人的資源の管理が感染制御に寄与し得ることが示唆される。

看護師の過重労働は、手洗いコンプライアンスの低下とコホーティングの実施率低下をもたらすリスクが高まるため、常に過重労働を避けるような人員配置が必要である。英国では、各部署への人員配置については、病院の特性に合わせて、フレキシブルに病院の予算内で決定している。
