Deep wound infection after proximal femoral fracture: consequences and costs

本研究の目的は、近位大腿骨骨折手術後の深部創感染による患者の死亡率および社会的影響、およびNational Health Serviceへの経済的負担の影響を検討することである。6年間で、近位大腿骨骨折61例に深部手術創感染の合併症が起きた。これらの症例を、感染のない患者群122例と、これにマッチングさせた対照群と比較した。感染症例では入院期間が大幅に延長し（P＜0.001）、生存退院率は4.5分の1に減少し（P＝0.002）、たとえ生存しても元の住居への帰宅率は3分の1に減少した（P＝0.05）。感染症例あたりの治療費総額は24,410ポンド、対照では7,210ポンドであった（P＜0.001）。メチシリン耐性黄色ブドウ球菌（MRSA）感染では、非MRSA感染と比較して入院期間と費用が増加した（P＝0.02）。近位大腿骨骨折後の深部創感染は、患者および医療サービス提供者の両方にとって、甚大かつ不経済な合併症である。この社会的脆弱グループにおける感染リスクを最小限にするための適切な対策費用を確保し治療費を拠出するために、外傷部門へ予算を配分する場合には経済的影響まで考慮すべきである。

監訳者コメント：
マッチングによるケースコントロール研究を用いた小規模な研究である。しかし統計学的なエビデンスが強く、明らかに病院感染症が社会的にも甚大な被害をもたらしているということを強く訴えかけるメッセージをもっている。
