Incidence of hospital-acquired infections in Italian long-term-care facilities: a prospective six-month surveillance

本研究は、イタリアの4カ所の長期ケア施設で実施した、病院感染の6カ月間の前向きサーベイランスの結果である。859例の患者を組み入れ、21,503例・日の観察を行った。254件の病院感染が188例の患者で発生した。全感染率は1,000例・日あたり11.8であった。最も頻度の高い感染は、尿路感染（1,000例・日あたり3.2）、下気道感染（1,000例・日あたり2.7）、および皮膚感染（1,000例・日あたり2.5）であった。多変量回帰モデルによる病院感染に関連する危険因子（リスクファクター）は、在院期間28日以上［オッズ比（OR）3.5、95％信頼区間（CI）2.4～5.0］、医療機器の挿管（OR 2.0、95％CI 1.3～3.0）、Nortonスケール※（褥瘡発生予測スケール）12未満（OR 1.8、95％CI 1.2～2.6）、および寝たきり状態（OR 1.7、95％CI 1.08～2.6）であった。病院感染の存在により、在院期間中央値が増加したが（31日対20日、P＜0.01）、致死的な転帰には有意な影響がなかった（OR 1.4、95％ CI 0.7～2.7）。
