An outbreak of Serratia marcescens on the neonatal unit: a tale of two clones

セラチア（Serratia）属菌はハイリスク施設における病院感染と集団発生の重大な起因菌である。2001年から2002年の9カ月間、ある新生児室では患者21例が感染あるいは保菌していた。採取した22分離株に対して抗生物質感受性、β-ラクタマーゼ産生、および遺伝子型の検査を行った。Random-amplified polymorphic DNA-PCR（RAPD-PCR）法とパルスフィールドゲル電気泳動法により、2種類のクローンの存在が明らかとなった。第一のクローンは新生児4例の臨床的な侵襲性感染症の原因となり、その後は非侵襲性クローンと交代して新生児14例が感染した。表現型も、2種類の菌株はプロジギオシンの産生の点で異なっていた。第一の菌株は無色であったが、第二の菌株はピンクレッドの色素産生を示した。臨床的特徴から病原性の相違が示唆された。環境中の感染源は見いだされなかった。感染制御対策の遵守を強化し、抗生物質の投与方針を変更した後、集団発生は終息した。S. marcescensは追跡調査の5カ月間にしばしば分離されたが、これらは異なる遺伝子型をもつ散発性の分離株であった。

毒をもって毒を制す。プロジギオシンはセラチア菌のコロニーが赤くみえる原因となる物質である。わが国の病院で分離される多くのセラチア菌は最近、このプロジギオンを産生しない。
