ペン型注射器による針刺しリスク ★★

ペン型注射器は、皮下への自己注射による薬物投与（インスリン、インターフェロン、アポモルフィンなど）で、患者が使用している。本研究の目的は、医療従事者のペン型注射器の使用率とそれに伴う針刺しリスクを評価して、ペン型注射器と皮下注射器の針刺し発生率を記録、比較することとした。
フランスの24の拠点公立病院で1年間の後向き研究を実施した。1999年10月から2000年9月の間に、医療従事者が所属の診療科に自発的に報告した皮下注射処置に関連するすべての針刺しを、標準化された質問票を用いて記録した。同期間に追加データ（報告された針刺し件数、購入された皮下注射器数）を収集した。
皮下注射に伴う合計144件の針刺し事故が報告された。ペン型注射器での針刺し発生率は使い捨て注射器による発生率の6倍であった。ペン型注射器での針刺しは皮下注射に関連する針刺しの39％を占めた。全体では、ペン型注射器での針刺しの60％が器具の分解に関連していた。
ペン型注射器に伴う針刺しは注射針の6倍の頻度であった。それにもかかわらず、ペン型注射器は患者の治療の質を改善することが示されており、治療遵守を向上させると考えられる。本研究はペン型注射器安全器材の必要性を示している。
監訳者コメント：
ペン型注射器は患者の自己注射のために利用されるが、医療従事者にとっては針刺しのリスクが高く、注意が必要である。著者も述べているように、医療従事者の針刺しのリスクを軽減するデバイスの開発・普及が待たれる。
