Control of a methicillin-resistant Staphylococcus aureus (MRSA) outbreak in a day-care institution

本論文では、コペンハーゲンの2つの重複障害児施設における、メチシリン耐性黄色ブドウ球菌（MRSA）の集団発生について報告する。本研究の目的は、高度の身体的接触があった重複障害児と職員という環境下で、MRSAの根絶が可能かどうかを判定することである。本研究は、2003年１月から2005年3月に行われた前向き介入非対照コホート研究である。2つの施設を頻繁に来訪した者または集団発生でのMRSA保菌者と密接に接触した者全員を本研究の対象とした。対象者は患児38例、職員60名、および保菌者の近親者12名であった。感染制御対策として、全員のスクリーニングを実施した。MRSAの感染または保菌が認められた場合は、MRSAの除菌を試み、職員の教育を実施し、伝播経路の特定を試みた。11例がMRSA陽性であった（10.0％）。すべての分離株は、パルスフィールドゲル電気泳動法で同一であることが示され、ブドウ球菌カセット染色体mec（SCCmec）IV型を有していた。すべての保菌者と感染者は、一方の施設の１つの部屋と関係があった。すべての保菌者と感染者のMRSAを除菌した。本研究は、高度の身体的接触がある重複障害児施設におけるMRSAの集団発生を制御することが可能なことを示している。

監訳者コメント：
市中感染型MRSAは人口密度が高かったり、皮膚どうしが擦れ合い切り傷を負うような環境下で皮膚軟部組織感染症を引き起こすことが知られている。本事例のような施設でも該当するリスクが整っており、施設内伝播したものと考えられる。基本は皮膚の衛生管理である。
