High prevalence of nosocomial infections in rehabilitation units accounted for by urinary tract infections in patients with spinal cord injury

本研究（2001年にフランスで実施された全国規模の罹患率サーベイランスの一部）は、リハビリテーション病棟入院患者における院内感染の罹患率と危険因子（リスクファクター）、および耐性菌叢の調査を目的として計画した。Hospices Civils de Lyonグループの2カ所の病院286例の患者を対象とした。患者は脊髄損傷の有無により分類した。78例（27.3％）に脊髄損傷があった。これらの患者は若齢で、日常生活動作スコアが低い場合が多く、尿失禁と慢性呼吸器疾患が多かった。導尿カテーテルおよび人工呼吸器の利用頻度が、これらの患者では高かった。院内感染罹患率は脊髄損傷群で高く（21.8％対4.3％、P＜0.00001）、特に尿路感染で顕著であった（19.2％対3.4％、P＜0.00001）。危険因子の数と院内感染罹患とは正の関係があった。多変量解析では、院内感染罹患の唯一の独立した危険因子は導尿カテーテルの留置であった［オッズ比（OR）11.64、95％信頼区間（CI）2.53～53.65、P＝0.002］。慢性腎疾患または慢性肝疾患（OR 5.84、95％CI 0.80～42.68、P＝0.082）と脊髄損傷（OR 2.97、95％CI 0.61～14.60、P＝0.179）は、有意性の周縁域の因子であった。抗生物質耐性菌の罹患率は高かったが（感染部位31に対して耐性菌9例）、患者群による相違はなかった。脊髄損傷患者における高率の院内感染、特に尿路感染は、脊髄損傷の独立した影響によるものではなく、危険因子が多いことに起因する可能性が高い。これらの高リスク患者へは、院内感染の集中的なサーベイランスが必要である。
