本稿では、英国南部の4病院において実施された、それぞれの人工骨頭置換術（American Association of AnaesthetistsグレードⅣ以上）において、2種類の洗浄方法のうちのひとつを無作為に割り付けた、前向き無作為臨床試験について報告する。第一群の患者は生理食塩水2 Lのパルス洗浄※を、第二群は生理食塩水2 Lのピッチャーまたは注射器による洗浄※※を受けた。すべての創部の観察を、術後の入院中には30日まで、あるいは退院時まで行った（Nosocomial Infection National Surveillance Surveyの基準を採用）。感染により再入院した患者はすべて記録した。パルス洗浄群では全感染率が有意に低く、特に“関節裂隙”または深部感染率が減少した。

整形外科領域で一般的に信じられている、The solution to pollution is dilution（汚染の解決法は希釈することである）ということわざがあるが、それを実証するエビデンスのひとつとなる論文である。ただし、糖尿病や肥満などの危険因子（リスクファクター）で階層化はしていない。
