整形外科的なインプラント手術における術中細菌汚染を減少させる対策の評価

Evaluation of measures to decrease intra-operative bacterial contamination in orthopaedic implant surgery

本研究の目的は、処置手順や総合的な対策により、人工股関節全置換術または人工膝関節全置換術の術中汚染が減少するかどうかを評価することである。また、18カ月の追跡調査期間中に、これらの対策後の創部滲出液の遷延、表在性手術部位感染、および深部人工関節周囲感染も調査した。207回の手術の開始時と終了時に手術器械から4カ所のスワブを採取した。さらに、除去した骨片(股関節の場合には寛骨臼および大腿骨、膝関節の場合には大腿骨および脛骨)の汚染の有無を検査した。最初に、従来の対照条件下での手術を70回実施した。まず行動上の対策(例えば、プレナムのより適切な使用)を導入し、67回の手術中に培養検体を採取した(第1群)。続いて、作業訓練による対策および層流システム設置を行い、この第二の介入後に70回の手術をモニターした(第2群)。術中汚染が認められた症例は、対照群32.9%(23/70)、第1群34.3%(23/67)、第2群8.6%(6/70)であった。第2群では、創部滲出液の遷延および表在性手術部位感染は、深部人工関節周囲感染の発生率とともに著明な減少を示したが、深部人工関節周囲感染には統計学的有意差は認められなかった。本研究は、手術室において総合的な対策と作業訓練による対策を組み合わせることにより、術中の細菌汚染、その後の創部滲出液の遷延、および表在性手術部位感染の発生が著明に減少することを示した。また、18カ月の追跡後には、深部の人工関節周囲感染の減少も認められた。
